Your First Visit
Discover all of the information you need to prepare for your upcoming visit at any of our 8 locations.
With fifteen dermatologists, nine physician-assistants, a nurse practitioner, three fellowship-trained mohs surgeons, two plastic surgeons, two aestheticians and 8 locations, our staff will make every effort to accommodate your schedule. If you cannot make your appointment, please call our office 24 hours in advance to cancel or reschedule.
Center for Dermatology & Plastic Surgery participates in a large number of insurance plans; however, you will need to verify our participation with your particular plan before you come in for an appointment.
On your first visit for our medical dermatology services, we will examine your skin condition, review your medical history and answer your questions.
What to Bring
- Completed patient information forms
- Name, address and phone number of your pharmacy
- Insurance card(s)
- Driver’s license
- Any previous medical records, especially test results
Arrive 10 minutes early
Please review directions to our building and plan to arrive 10 minutes before your appointment. Please arrive 30 minutes ahead of your appointment if you need to fill out your paperwork at the office.
